Information Requirements

In order for us to properly quote on your request we will need the following information.

• Equipment to cover: Select the item type your wish to have covered. Specifics can be added in the "Additional design notes" section.
• Material required: We stock the two material types shown. We can quote on most materials.
• Item dimensions: Please provide the approximate Height, Length and Width of the item. Please include the feet and inches.
• Additional design notes: Please use this section to provide additional details about the item to help us provide you with our quote.
• Upload: If you have a design file (can be a photo of the artwork, raw material, finished item, drawing) that can be uploaded from your computer, please use our form's upload option. It is not a requirement for the quote.

After form submission a copy will be emailed to you. Please make sure your email address is entered correctly.
